The applicant will also need to secure a location inspection report.

  4. Electrical Permit/Connection Order
  5. Occupancy Permit/Building Permit
  6. Right of Way Easement (Attached copy of Tax Declaration/Title)
  7. Marriage Contract (Joint Membership)
  8. Any government-issued ID (SSS/GSIS/Passport/Driver’s License) or cedula. Bring photocopy.
  9. Death Certificate (Photocopy for Change Name of Account due to death)
  10. Deed of Sale (Photocopy with Certified True Copy from original)
  11. Waiver (For Change Name of Account)
  12. Deed of Joint Undertaking (Tenant, Attach Lease Contact-Photocopy)
  13. Statement of Account/Receipt (Change Name of Account)
  14. Special Power of Attorney (Representative)
  15. Duly Accomplished Application Form
